We are open Monday through
Thursday and many Saturdays throughout the school
year.
Your appointment time
is scheduled and reserved for you. We request that you
give our office at least 24 hours notice of
cancellation. The first time we wave a fee because
we are aware that sometimes things that are
unavoidable can happen. After missing your
appointment for a second time you are subject to an
additional charge.
There is also a fee to replace
broken or lost retainers or orthodontic appliances.
We can only estimate your individual insurance
coverage but cannot guarantee your coverage due to
the complexity of insurance contracts. Therefore if
insurance fails to render payment you will be
responsible for the entire balance.
As a service and courtesy to
our patients we will bill insurance carriers for you
and try to be of assistance. If during treatment
your insurance coverage benefit changes in any way
you are responsible for all charges.
